The research institution should also keep records on veterinary preventative medicine procedures, treatments, animal maintenance practices and research procedures. The animal health record should include the date of euthanasia or other disposition of the animal.
Each institution that conducts research or testing with animals must file an annual report with the U.S. Department of Agriculture. Information and forms can be obtained from the USDA Veterinarian in Charge in the capital city of your state.
